目的

本研究拟总结大样本量机器人胰腺手术助手的经验,对早期参加机器人胰腺手术的手术助手起到教学作用。

方法

2011~2021年,笔者所在团队完成机器人胰腺手术共2 835例,其中胰十二指肠切除1 420例、胰体尾切除1 005例、胰腺肿瘤剜除205例、胰腺中段切除119例、其他86例。结合术中操作体验以及所遇问题,不断优化总结出作为机器人胰腺手术助手的经验。另外,采用累积和方法对机械臂连接时间的学习曲线进行定量评价。

结果

从术前准备、体位选择、Trocar布孔和机械臂连接、术中配合技巧4个方面总结了机器人手术助手在胰腺手术过程中的经验。通过分析前50例的机械臂连接时间的学习曲线,发现在17例后,机械臂连接时间明显下降。

结论

本研究是从临床实践中总结出经验,提出了各项操作原则和基本方法,希望能够为胰腺外科医师成长为合格的机器人手术助手提供教学示范作用。

Objective

This study aims to summarize the experience of a large sample of robotic pancreatic surgery assistants, and to serve as a teaching tool for assistants who participate in robotic pancreatic surgery at the early stage.

Methods

From 2011 to 2021, our team performed a total of 2 835 robotic pancreatic surgeries, including 1 420 robotic pancreaticoduodenectomies, 1 005 robotic distal pancreatectomies, 205 robotic pancreatic enucleations, 119 robotic central pancreatectomies, and 86 others. Combined with the operation experience and problems encountered during the operation, the experience as a robotic pancreatic surgery assistant was continuously optimized and summarized. In addition, the cumulative sum method was used to quantitatively evaluate the learning curve of the robotic arm docking time.

Results

In this study, the experience of robotic surgical assistant in pancreatic surgery was summarized from four aspects: preoperative preparation, position selection, Trocar placement and docking, and intraoperative cooperation skills. By analyzing the learning curve of docking time in the first 50 operations, it was found that the docking time decreased significantly after 17 operations.

Conclusions

This study summarized the experience from clinical practice, and put forward various operation principles and basic methods, hoping to provide a teaching demonstration for beginners to grow into qualified robotic surgical assistants.
