胆管癌是来源于胆管上皮的一组恶性肿瘤,根据原发肿瘤不同的解剖位置可将其分为肝内、肝门和远端胆管癌(distal cholangiocarcinoma,DCCA)。胆管癌在全世界的总体发病率呈增加趋势;由于临床表现缺乏特异性,多数胆管癌患者一经确诊已进入疾病进展期或晚期,加上缺乏疗效确实的化学药物治疗,患者总体预后仍远不令人满意[1,2]。通过根治性手术切除获得阴性切缘(R0切除)可能是目前最有效的治疗方法。研究报道:无法手术切除肿瘤的晚期胆管癌患者预计在诊断后1年内死亡,而根治性手术切除肿瘤后的胆管癌患者5年生存率达25%~40%;在影响胆管癌患者生存预后的众多因素中,除了胆管癌肿瘤分期和生物学特点以外,是否能通过根治性手术切除肿瘤获得阴性切缘(R0切除)是影响胆管癌患者生存预后最重要因素[3,4,5]
