手术切除仍然是肝脏肿瘤最有效的治疗手段。1991年,Reich等[1]完成世界首例腹腔镜肝脏良性肿瘤切除,标志着微创肝胆外科的开始。目前,腹腔镜肝切除技术越来越成熟,已广泛应用于肝脏良、恶性肿瘤的治疗。与开腹手术相比,腹腔镜手术具有术中出血量少、并发症发生率低、恢复快的特点,但其固有的缺陷限制了其在复杂肝胆手术中的应用;例如:手术视野不稳定、器械灵活度差、二维成像缺乏深度感知、不符合人体工程学设计导致术者疲劳等[2,3]。近年来,机器人手术系统作为一项新兴技术被引入外科领域,其具有7个自由度的机械臂、提供三维立体稳定的手术视野、滤过术者手部震颤、术者舒适度高不易疲劳,使得复杂肝胆疾病的微创外科治疗成为可能[4]。笔者综合国内外文献,结合笔者团队经验,深入探讨机器人手术系统在肝脏肿瘤治疗中的应用现状及前景。
